Launched in 2024 in Flint, Michigan, Rx Kids is the nation’s first community-wide prenatal and infant cash program. Here’s how it works: every pregnant person in a participating community receives $1,500 — no strings attached — during pregnancy, followed by $500 per month throughout their baby’s first year of life. No gatekeeping. No conditions. Just trust that parents know what their families need.
And the outcomes? Extraordinary.
A citywide study found that Rx Kids cut preterm births by up to 18% and low birthweight births by up to 27% — preventing an estimated 60 NICU admissions and saving $6.2 million in healthcare costs. A separate survey of 1,000+ moms found the program nearly eliminated evictions and reduced housing debt. Families used the money for baby supplies, utility bills, food, and transportation — the basics that make recovery possible. Every dollar invested in Flint generated an estimated $1.60 back into the local economy.
The program has since expanded to nearly a dozen Michigan communities, delivering more than $14 million directly to families. States across the country are now reaching out to replicate it.

Following the LA wildfires, 4th Trimester distributed nearly $200,000 in direct cash assistance to postpartum families who had nowhere else to turn. Claudia, a cash assistance recipient from Altadena, put it simply:
“The cash aid was instrumental in helping us move forward after the Eaton fires. We used the money to help replace baby items that had been smoke-damaged. Our baby came in February, and we had everything she needed to thrive. So many tears of gratitude have been shed.”
And Caroline, also from Altadena: “This gift is going to go a long way in helping me recover. I can’t wait to get back on my feet and pay it forward.”
